Our referral process is simple and confidential—designed to connect patients with the right level of support across medical, behavioral, and recovery services.

Absolutely. All referrals are processed according to HIPAA and 42CFR Part 2 Confidentiality laws. Only authorized care staff will have access to referral information.

Yes. Our team provides confirmation and follow-up communication to ensure seamless coordination. Partners can also contact our care coordination department for status updates.

For immediate behavioral or medical crises, please direct patients to 911 or the local crisis line. You may still submit a referral for follow-up care once the patient is stabilized.

Yes. We serve all community members, including those without insurance or stable housing. Our staff can assist with Medicaid enrollment or connect patients to community resources.
